FAQs About Neurotoxins: Botox®, Dysport® and Xeomin®

Botox®, Dysport® and Xeomin® are all brand-name medications that temporarily weaken muscular activity when injected under the skin and into the specific muscle. These medicines are different patented formulations of a purified protein called Botulinum Toxin-A. They all work by chemically disabling the communication between the nerve and muscle, and are used cosmetically to reduce dynamic wrinkles—particularly in the upper face. 

Q: Do the injections hurt?

A: Well, they are injections, and they do go into your face—with a needle. But the providers at Morpheus are extremely gentle and kind. While some patients like us to apply ice to their skin and numb it beforehand, others say they can barely feel the injections.

Q: Which wrinkles can be treated with neurotoxins?

A: Patients often come in and ask if they can have their smile lines treated with Botox. The answer is no! One of the most important things to understand about neurotoxins is that they can only be used to treat dynamic lines created by expressions and movements—which are okay to eliminate. Smile lines are dynamic, but they are not okay to eliminate. 

The deep frown lines that occur between the eyebrows (also called furrows or “11s”) are perfect for this treatment because, to quote Dr. Chilcott, “Nobody needs that face!” The expert injectors at Morpheus will provide every client with an individual treatment plan that is specific to their face and goals. Q: What are the differences among Botox, Dysport and Xeomin?

A: Dr. Chilcott is fond of saying (jokingly, of course) that the primary difference between the three brands of neurotoxins is spelling. Like Kleenex®, Scott® and Cottonelle®, there are subtle differences in the product (on a molecular level), but the most notable differences are in market awareness. There is also a small difference in cost, with Botox priced at $16 per unit and Dysport and Xeomin priced at $15 per unit (for the equivalent amount). These products last about three months, have the same effect on muscles and are injected in the same manner.

Q: What is the discussion about offices “diluting” their products and charging less? 

A: All cosmetic botulinum toxin products are shipped to medical offices from the manufacturers in a dry powder form. This means that all offices “dilute” the product, as they must reconstitute it to prepare it for patient injections.

The difference is in how much sterile saline is used to dilute the product per vial, since the amount of saline used changes the concentration of the product within each syringe. If a provider uses 5 ccs of saline in a 100-unit vial, there will be 20 units per cc. If a provider uses 2 ccs of saline to reconstitute a 100-unit vial, there will be 50 units per cc. At Morpheus Medical Aesthetics, clients receive the exact amount of product they need measured in units, not in ccs or mls. Our standard is that we reconstitute with 2 ccs of preserved sterile saline per 100 units of product.

Q: How much of each neurotoxin do I need?

A: The amount, or dose, of Botox, Dysport or Xeomin you need will depend on the size and strength of the muscles being injected and your desired results. If your muscles are well-developed and you want to go from very active expressions to stillness, this will require much more medication than if you had weaker muscles and only want to soften your expression. That said, a typical dose used to treat the glabellar area (between the eyebrows) is 20 to 40 units. Crow’s feet typically require four to 15 units per side, and for the upper forehead, we use half as much.
